Total Life Care, PLLC began January 1989 as a solo family practice office. It has grown into a combined family practice and internal medicine care center featuring four female physicians Dr. Lee Tolar Meals, Dr. Karen C. Banister, Dr. Rheanel A. Tolar and Dr. Jennifer W. Nelson. Our focus is to provide comprehensive medical care with compassion and kindness.
 
Due to patients concerns with aging and weight control, we are happily introducing a new division of Total Life Care, Ageless Beauty Medical Spa. This facility is designed to enhance our patients self-esteem and natural beauty through advanced medical treatments.
 
 

Dr. Lee Tolar Meals

Dr. Lee Tolar Meals grew up in Paducah, Kentucky. According to her family, she wanted to be a doctor even when she was a child. Lee attended Murray State University and graduated with a degree in biology. After moving to Louisville, Kentucky for medical school and residency, she met and married Mike Meals. Dr. Lee Meals works as an Internal Medicine physician in Lone Oak, Kentucky. She enjoys celebrating the Chinese New Year with her daughter Audrey and watching her older daughter Noelle dance and act.

Dr. Karen Banister

Dr Karen Banister grew up in Reidland, outside Paducah, Kentucky. As a child she watched her Father, Dr. Bill Cummings, a veterinarian, take care of animals. This inspired her drive into the medical field. She currently lives and works in Lone Oak as a Family Practice physician. Karen attended Murray State University where she majored in exercise science and biology. She met Jason Banister here and they both went on to medical school at the University of Louisville. She completed her Family Practice residency at St. Elizabeth’s Medical Center in northern Kentucky. Her interests include weight lifting and spending time her three children Sarah, David, and Samuel.

Dr. Rheanel Tolar

Dr. Rheanel Tolar grew up in rural Missouri. She moved to Kentucky her junior year of high school and graduated at Logan County High School in Russellville, Kentucky. She started as an art major at Murray State University and completed three years of art training before changing majors to biology, completing the prerequisites for medical school. It was there, in biology class, that Dr. Tolar met the man she would later marry, Stewart Blair Tolar. In 1996, Rheanel moved to Louisville, Kentucky. She started medical school at the University of Louisville, the following year. The year 2000 found Blair completing medical school and he and Rheanel married. She graduated the following year and started her internship at the University of Louisville Hospital in Internal Medicine. Not even Blair’s career in OB/GYN could prepare the couple for the birth of their twin boys in July of 2004. Noah and Colin were born several weeks early but were healthy and strong. When the boys were ten days old the family moved to Paducah, Kentucky where Rheanel and Blair practice medicine. Rheanel joined Dr. Lee Meals and Dr. Karen Banister at Total Life Care. After the twins turned two years old, Blair and Rheanel decided to add to the family and welcomed Max in October of 2007. The family enjoys church at the Reidland Church of Christ. Dr. Tolar enjoys scrapbooking, playing with her children, and taking photographs.

Dr. Jennifer Nelson

Dr. Jennifer Nelson currently lives in Paducah and works as a Family Practice doctor in Lone Oak, Kentucky. She grew up and graduated high school in Marshall County where her mother, Sandra still teaches school. Jennifer attended Murray State University and majored in biology. She went on to complete her medical training in Pikeville, Kentucky. Trent Nelson D.M.D. and Dr. Jennifer Nelson were married in 1999 and have one child, Jack. Current interests of hers include, shopping for toys for Jack and playing with her two dogs.

Sara C Berry, RD, LD

Graduated Magna Cum Laude from Saint Louis University in St. Louis, MO with a Bachelor’s of Science in Nutrition and Dietetics in 2005. I then completed a 10 month Dietetic internship through Murray State University at Lourdes Hospital in 2006. I am presently pursuing a Master of Nutrition Sciences at Murray State University. I have been a Registered and Licensed Dietitian since 2006.

I have been a long-term believer in the benefits of a healthy lifestyle through diet and exercise. Since early in my high school education, I have pursued opportunities to help others in the wellness arena. In doing so, I also obtained my Certified Personal Trainer license through the National Strength and Conditioning Association. I use this certification to help my clients find activities that suit their abilities and their interests. As a Registered Dietitian and Wellness Coach, my goal is to help people realize that healthy food and a healthy lifestyle is something that is achievable and enjoyable.

Amy Crawford - Spa Coordinator

Amy Crawford is currently the Spa Coordinator and a Medical Assistant at Ageless Beauty Medical Spa. She brings sixteen years of sales experience to Ageless Beauty and is microdermabrasion and laser certified. She completed a real estate degree in 1997 and went on to complete a business and marketing degree at Madisonville Community College. She won the Championship for Compound Bow Archery in 2006 and is an active member of the Disabled American Veterans Auxiliary. Amy enjoys spending time with her two children Seth and Sarah.
